Finally added the copyright statement on all source code files and the test to ensure it is kept up to date

Needed for Docker swarm

Workers dying in SUBMITTED state are detected on in dumb Meadows using a built-in max_limbo_seconds timeout

schema_change: preregister Workers upon submission in SUBMITTED state, added when_submitted, relaxed cause_of_death to VARCHAR

bugfix: make sure TEXT field does not have a default value in MySQL (treated as a bug in TRADITIONAL mode)

schema_change: new implementation of Semaphores as separate objects that can cross pipeline database boundaries

This can't be easily patched in sqlite

patchfix: sqlite has a single global space for index names, so we need to drop the old ones to avoid collisions

patchfix: in PostreSQL, an enum value is not equal to its text counterpart, so the default value has to be changed
